COLLECTION OF PERSONAL INFORMATION
We most commonly collect Personal Information from members of the Coalition Loyalty Program during enrollment and rewards redemption processes. This Personal Information would generally comprise such details as business names, personal names, address, contact information details (including address, phone and email) and business segment information.
We may collect your Personal Information directly from you (usually from this Web Site), or from third parties including our related entities, contractors, agents or Sponsors. Demographic and other profile information beyond that gathered during enrollment may also be requested via optional surveys or other site promotions. This information will be used to deliver relevant, targeted offers to members and is not required to use the Web site.

USE OF PERSONAL INFORMATION
Member information collected is used to create and administer your Food Service Rewards® account, including for purposes of account verification if a member loses or forgets the Coalition Loyalty Program password. Food Service Rewards® will use this information to contact a member via e-mail, phone, or print communications regarding special Coalition Loyalty Program promotional offers featuring one or more Coalition Loyalty Program Sponsors. In addition, Food Service Rewards® may use this information to contact a member regarding any issues that may rise in the operation of the Web site or Coalition Loyalty Program. Food Service Rewards® does not sell or rent your personal information to any third parties.
If a member redeems Food Service Rewards® points for merchandise, the member may, at Food Service Rewards®’ discretion, receive communications with information about the transaction (such as order or shipment confirmation) or special promotions.
Use of personal information by third parties in conjunction with the operation and management of the program is restricted to necessary functions of the Coalition Loyalty Program and is limited to the least amount of information necessary for that third party to perform its function. As an example, your name and address MAY need to be shared with a third party shipping company.
The Coalition Loyalty Program collects and analyzes aggregate level data derived from the Coalition Loyalty Program that does not include personally identifiable information of any member. Aggregate level data may be used by the Coalition Loyalty Program for any legal purpose.
INFORMATION PROVIDED TO SPONSORS
When a member enrolls in Food Service Rewards®, a member completes an enrollment profile. Information collected from this profile may be shared with all sponsoring manufacturers.
Member pricing data may be reported to Coalition Loyalty Program Sponsors once legally compliant requirements are met which currently means: (a) pricing data shared is more than three (3) months old, (b) a minimum of five Members’ data is collected, (c) no Member business accounts for more than 25% on a weighted basis of the data reported, (d) data aggregation prevents identification of a particular member’s pricing data.
When a member logs onto the Internet, a member is assigned an IP (Internet Protocol) address by an ISP (Internet Service Provider). Web sites can determine from what IP address a member is entering the site. Food Service Rewards® uses the IP address to help diagnose problems with our server and for security. Food Service Rewards® reserves the right to share aggregate ISP information with our Sponsors.
The Food Service Rewards® site utilizes "cookies" to improve your experience on our site. Cookies are small pieces of important information that are stored by your browser on your computer's hard drive. These are used to enhance site usability and personalization and to assist in site navigation. Our cookies contain only information that a member has supplied to, and can only be accessed by, this site. They cannot read other data off of your computer or share information with cookies from other sites. A cookie may remain on your computer after your browser session has expired to assist a member when a member returns to this site.
Occasionally Food Service Rewards® may place ads on the Web site. These ads may also contain cookies and appear to be coming from our site, but are, in fact, coming from Sponsors or other parties who are presenting ads on our site.
You can configure your browser to accept all cookies, reject all cookies, or notify a member when a cookie is set. Each browser is different, so check the "help" menu of your browser to learn how to change your cookie preferences. If a member rejects all cookies, a member may not be able to use some or all of the features of foodservicerewards.com.
The Food Service Rewards® site has security measures in place to prevent misuse, fraud and alteration of the information under its control. All personal and account activity information collected and used by Food Service Rewards® is protected via user name and password and is not accessible by the public or via the Internet. Food Service Rewards® restricts access to personally identifiable information gathered through the Web site to program employees who need access to the information for the purposes of administering the Food Service Rewards® Program. These employees are limited in number, and are committed to Food Service Rewards®' privacy and security policies. Food Service Rewards® takes the extra step of having our employees execute comprehensive nondisclosure agreements with us to protect member's confidential information.
REVIEW AND CHANGE OF INFORMATION
A member can review and change portions of your personal information in the Account Profile area of the Web site. While Food Service Rewards® makes reasonable efforts to ensure data integrity to the extent that some information may have been transferred to other databases not directly administered by foodservicerewards.com, Food Service Rewards® cannot ensure the timeliness with which such corrections or deletions may reach other databases.
