Foodservice Rewards® (www.foodservicerewards.com) ("Coalition Loyalty Program") and its Sponsors (industry manufacturers which are sponsors of the Foodservice Rewards coalition) are very sensitive to the privacy concerns of visitors to this loyalty program Web site. The purpose of this Online Privacy Policy is to describe what types of information Foodservice Rewards collects from users and Coalition Loyalty Program members, the purposes for which Foodservice Rewards uses the information and under what circumstances Foodservice Rewards discloses information gathered. Please note, by participating in this Coalition Loyalty Program and using the Web site, members agree to a limited use of the personal information members provide, as outlined below.

COLLECTION OF INFORMATION

Members in Foodservice Rewards are required to provide business name, business type, contact information (including address, phone and email) and business segment information. Phone number is required in the event that Foodservice Rewards needs to contact a member if there is a problem with an order.

Demographic and other profile information beyond that gathered during enrollment may also be requested via optional surveys or other site promotions. This information will be used to deliver relevant, targeted offers to members and is not required to use the Web site.

We adhere to the Safe Harbor principals and have made a commitment to cooperate with Data Protection Authorities located in the European Union or their authorized representatives.



USE OF PERSONAL INFORMATION

Member information collected is used to create and administer your Foodservice Rewards account, including for purposes of account verification if a member loses or forgets the Coalition Loyalty Program password. Foodservice Rewards will use this information to contact a member via e-mail or print communications regarding special Coalition Loyalty Program promotional offers featuring one or more Coalition Loyalty Program Sponsors. In addition, Foodservice Rewards may use this information to contact a member regarding any issues that may rise in the operation of the Web site or Coalition Loyalty Program. Foodservice Rewards does not sell or rent your personal information to any third parties.

If a member redeems Foodservice Rewards points for merchandise, the member may, at Foodservice Rewards’ discretion, receive communications with information about the transaction (such as order or shipment confirmation) or special promotions.

Use of personal information by third parties in conjunction with the operation and management of the program is restricted to necessary functions of the Coalition Loyalty Program and is limited to the least amount of information necessary for that third party to perform its function.  As an example, your name and address MAY need to be shared with a third party shipping company.



INFORMATION PROVIDED TO SPONSORS

When a member enrolls in Foodservice Rewards, a member completes an enrollment profile. Information collected from this profile will be shared with all sponsoring manufacturers.

Upon enrolling, a member shall have the choice to opt-in to receive informative emails about sponsoring manufacturers' products and services directly from Sponsors. If a member chooses not to opt-in, Foodservice Rewards will not provide Sponsors with your email address. Subsequent to enrolling, a member may opt-in to receive Sponsor emails through the Foodservice Rewards Web site. The member will need to unsubscribe from communications received directly from Sponsors via the message's unsubscribe link.

ISP ADDRESSES

When a member logs onto the Internet, a member is assigned an IP (Internet Protocol) address by an ISP (Internet Service Provider). Web sites can determine from what IP address a member is entering the site. Foodservice Rewards uses the IP address to help diagnose problems with our server and for security. Foodservice Rewards reserves the right to share aggregate ISP information with our Sponsors.

COOKIES

The Foodservice Rewards site utilizes "cookies" to improve your experience on our site. Cookies are small pieces of important information that are stored by your browser on your computer's hard drive. These are used to enhance site usability and personalization and to assist in site navigation. Our cookies contain only information that a member has supplied to, and can only be accessed by, this site. They cannot read other data off of your computer or share information with cookies from other sites. A cookie may remain on your computer after your browser session has expired to assist a member when a member returns to this site.

Occasionally Foodservice Rewards may place ads on the Web site. These ads may also contain cookies and appear to be coming from our site, but are, in fact, coming from Sponsors or other parties who are presenting ads on our site.

If a member rejects all cookies, a member may not be able to use some or all of the features of foodservicerewards.com.

SECURITY

The Foodservice Rewards site has security measures in place to prevent misuse, fraud and alteration of the information under its control. All personal and account activity information collected and used by Foodservice Rewards is protected via user name and password and is not accessible by the public or via the Internet.

Foodservice Rewards restricts access to personally identifiable information gathered through the Web site to program employees who need access to the information for the purposes of administering the Foodservice Rewards Program. These employees are limited in number, and are committed to Foodservice Rewards' privacy and security policies. Foodservice Rewards takes the extra step of having our employees execute comprehensive nondisclosure agreements with us to protect member's confidential information.

REVIEW AND CHANGE OF INFORMATION

A member can review and change portions of your personal information in the Account Profile area of the Web site. While Foodservice Rewards makes reasonable efforts to ensure data integrity to the extent that some information may have been transferred to other databases not directly administered by foodservicerewards.com, Foodservice Rewards cannot ensure the timeliness with which such corrections or deletions may reach other databases.

ACCEPTANCE OF PRIVACY TERMS AND CONDITIONS

By visiting www.foodservicerewards.com, members agree to the practices described in this Online Privacy Policy. To the extent that any of the third-party sites accessible through Foodservice Rewards have different privacy practices from those stated in the foodservicerewards.com Online Privacy Policy, those third-party privacy practices govern the collection and use of information a member provides when visiting those sites. Foodservice Rewards is not responsible for the policies, content and practices of other companies.

CHANGES TO THE FOODSERVICEREWARDS.COM ONLINE PRIVACY POLICY

On occasion, it may be necessary for Foodservice Rewards to change the terms and conditions of the foodservicerewards.com Online Privacy Policy. To ensure that a member is aware of current privacy practices, Foodservice Rewards recommends that a member check this site periodically. Foodservice Rewards will not materially expand how Foodservice Rewards handles previously collected information without providing notice and choice to opt-out before the policy is materially expanded.
